Diagnostic Value of vHIT, VOG and Rotational testing in differentiating Central and Peripheral Vestibular Diseases


This talk will explore the prevalence of vertigo, dizziness and unsteadiness in the growing elderly population. Technology is available today that allows us to better assess the origin of vestibular disorders so we can quickly differentiate central from peripheral lesions.  Video Head Impulse testing, Ocular Motor testing and Rotational Chairs are tools that can aid in the clinical diagnosis and lead us to quicker intervention. Typically a normal clinical HIT test in the presence of vestibular symptoms suggests a central lesion/stroke and when combined with ocular motor measurements and tests of skew deviation as in the HINTS test we can predict with high sensitivity the origin of the lesion. Sometimes we also see abnormal findings in HIT testing such as the presence of vertical corrective saccades or elevated gains which can sometimes predict cerebellar lesions. With lesions involving the vestibular nucleus, medial longitudinal fasciculus, and cerebellum we can sometimes see decreased or increased gains of the VOR during vertical HITs and we can confirm these findings with rotary testing. By defining the differences in patterns observed during these tests, we better localize the responsible lesions in both central and peripheral vestibulopathy.




VHIT、VOG和转椅试验在鉴别中枢性和外周性前庭疾病的诊断价值


伴随着人口老龄化的加剧,头晕、眩晕和站立不稳的发病率逐年攀升。今天快速发展的前庭功能检测技术,帮助我们能够更准确地评估前庭疾病的病因,以便快速鉴别前庭外周性损伤和前庭中枢性损伤。在临床诊断工作中,vHIT、眼动试验和转椅试验都是非常好的工具,能够帮助我们进行更精确的诊断并及早实施干预治疗。


代表性的HINTS测试(头脉冲试验、眼动试验、眼偏斜试验),对于鉴别早期中枢损伤/脑卒中有很高的敏感性。与此同时,在一些HIT测试中,如存在垂直矫正性眼跳或增益升高,则提示小脑损伤。部分存在前庭神经核病变、内侧纵向束病变或小脑病变的患者在垂直HIT的测试中,VOR的增益减少或增加,再对这类患者行转椅试验后,可以对病变部位进行确诊。通过不同的测试组合,我们可以更准确地对前庭中枢性和前庭外周性疾病进行定位诊断。
